Eligibility and Program Requirements - Employers

Loan reimbursement through MiLES is 100% state-funded and does not require employer contributions.

Qualified Practice Sites

MiLES-approved practice sites are health care facilities that generally provide outpatient primary or mental health services to populations residing in urban or rural Health Professional Shortage Areas (HPSAs) or Medically Underserved Area or Population (MUA/P). Any score is fine as long as the area or site has a designation. Psychiatry practice site locations must have a Mental Health HSPA designation.
Examples include:

  • Federally Qualified Health Centers (FQHCs)

  • Rural Health Clinics (RHCs)

  • Indian Health Service, Tribal or Urban Indian Health Clinics

  • Public Health Departments

  • Hospital-affiliated outpatient primary care practices

  • Solo or group private practices

  • Critical Access Hospitals (CAHs)

  • Indian Health Service Hospitals (IHS)

The following are not eligible MiLES practice sites, even if they are located in a HPSA or MUA/P:

  • County/local prisons

  • Inpatient Hospitals (except for CAHs or IHS Hospitals)

  • Other Inpatient Facilities

  • Clinics that limit care to veterans/active duty personnel (e.g. VA Medical Centers and Clinics, Military Bases, Civilian Health Care Providers in the Tricare Network)

The MiLES participant must be employed a minimum of 40 hours/week, for a minimum of 45 weeks each service year. The 40 hours per week may be compressed into no less than 4 days/week, with no more than 12 hours of work to be performed in any 24-hour period. At least 32 hours/week are spent providing patient care.

Employer Requirements
  • Have an interest in candidates likely to stay beyond 2 years

  • Have at least one open physician position in the eligible specialties (family medicine, internal medicine, pediatrics, OB-GYN, psychiatry, general surgery).

Employer Commitment

  • Offer a competitive salary and employment package for MiLES physician(s).

  • Post job opportunities with the MiLES program.

  • Complete an In-Service Verification (ISV) form every 6 months for each MiLES participant(s) you employ during their 2 year service commitment.
